The Transition Service supports young people with severe and enduring disabilities and their families through the process of moving from childhood to adulthood, and into Adult Services. Our main clients are those receiving a service from the Children’s Disability Team based at Sorrel Drive, although a small number of young people may be referred to the service via other routes, such as schools, GPs and family members. The team covers the whole of East Sussex. It has established links with schools, further education settings, and voluntary and charitable sector organisations.
